We accept referrals for adults (18+ y old) with the following cardiometabolic conditions:

  • hypertriglyceridemia and hypercholesterolemia
  • obesity and overweight state
  • pre-diabetes and insulin resistance

Referring physicians: Optionally, please complete and fax our referral form found here. The form also lists specific referral criteria for each of the above.

 

Patients: Once referred, please register to book an appointment here. By default your first appointment will be in person; follow-up appointments may be virtual.

 

We are located in The Armoury Sports Medicine and Performance Clinic. We offer appointment times Monday through Saturday. Wait times are currently < 1 month (as of 2025 Dec).
